We are excited to announce a full time Home Health Physical Therapist position based out of our brand new Willow Springs facility.We're excited to offer our PT a $5,000 signing bonus! Willow Springs in Tooele is a leader in skilled nursing care and rehabilitation servicesWe provide exceptional care for our patients and encourage family involvement during the rehabilitation process.We are looking for a physical therapist to serve our home health populationOur therapist will enjoy a generous hourly rate in addition to an extra $60-$70 paid per home health visitOur home health PT will need a current driver's license, reliable vehicle, and valid auto insurance policy.We're excited to offer our therapists an opportunity to deliver patient centered careWe empower therapists to decide what their patients need, and allow enough time in their day to deliverThere are no 15 minute encounters, we do not schedule at 125% production, and we encourage our therapists to spend the time to ensure each patient receives the care they require.Rocky Mountain Care is the only rehabilitation and skilled nursing community in Tooele, which means patients can remain close to their community during recovery.Our Physical Therapist will be responsible for the following:
Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons Diagnose movement dysfunction by listening to patients and observing them as they move about Develop an individualized treatment plan Coach patients and teach them therapeutic exercises and stretches to improve their condition Use hands-on therapy such as massage to ease patients' pain and provide stimulation to promote healing Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly Advise patients and their families on in-home treatment options and what to expect from the recovery processRocky Mountain Care at Willow Springs will patients quality care with our hometown charmOur Physical Therapist will enjoy a great team to work with, a brand new cutting edge facility to work in, endless career possibilities, and generous compensation.We are located at 80 East 2000 North, Tooele, UT 84074Our therapist will have the opportunity to see our resident patients as neededTake a virtual tour of our beautiful new Willow Springs facility at We look forward to meeting you!
